In the available literature is very poorly represented theoretical development, and the more illuminated side of the application of medical institutions in a competitive environment. It is clear that along with the other due to the fact that the national health system has not yet created the preconditions and the actual conditions for the implementation of medical activities in a competitive environment. Yes, and this environment is simply today's health care system, in form and content are mostly state does not exist. It is logical to assume that the emergence of competition, the practical manifestation of it in an environment of medical institutions (medical practices), the criteria and assessment of competitiveness - it is only possible to create conditions for a social environment that permits such a relationship. It is quite clear that the conditions to formulate and implement in medical institutions (medical practice) the essence of competitiveness, formed a specific set of management decisions at the level of state regulation, legislative initiatives, a number of administrative and managerial and organizational arrangements; in addressing the necessary research, economic and information problems, etc. Construction of the organization for optimal performance in a competitive environment should be provided with forms and methods of a high degree of formalization, models of management decisions and interventions, backed by science-based evidence, rather than the maxims. Structural-functional model of health care institutions (medical practice) as a subject of medical services market and, consequently, endowed with the function of competition, must embody the elements of the system, describing the organization is in terms of its competitiveness. We must recognize that, describing the medical establishment categories of "competition", "competitiveness", the researcher, primarily determines the interaction of such an organization competing with other actors, or relates the production of this organization (in this context - of health services) with similar products on the degree competitiveness. In classical theory, methodological approaches to assessment and management of competition and competitiveness in the organization generally developed quite deep. The annex to the health of the domestic medical institutions such research is very stingy, not counting some of the developments of foreign authors, mostly based on studies of their national health systems. Most likely, this state of research ideas related to the real lack of practical objects of economic health system, endowed with a competitive entity. However, starting the study, whose direction is stated in the title of this message, it should be emphasized that, by itself competitiveness as one of the entities functioning of the medical organization, is not absolutely crucial in the effective delivery of health care.
